Abstract
Workplace violence in healthcare settings is escalating nationwide and globally. Verbal de-escalation is a skill to diffuse situations with agitated and aggressive patients to decrease incidents of patient-to-staff injuries. Emergency department (ED) staff is a vulnerable population because of containment in a setting with mentally ill patients who may be unaware of their actions. The purpose of this study was to examine the differences between verbal de-escalation training and the emergency department staff knowledge to prevent patient-to-staff violence and injuries. This study was guided by the question: In emergency department nursing staff members, what is the effect of additional verbal de-escalation training, in comparison to current training on the confidence and knowledge to prevent patient-to-staff violence and injuries before and after the training? Based on the model for improvement framework, a quality improvement study with a survey design was used to examine the incidents of patient-to-staff violence after training is provided to emergency department nursing staff. The sample consisted of registered nurses in the selected EDs located in rural settings. The ED nursing staff received computer-based training on verbal de-escalation techniques. Twenty-eight peer-reviewed articles discussing workplace violence in healthcare and the effects of verbal de-escalation training have been reviewed and analyzed in this study. Verbal de-escalation promotes patient and staff safety leading to positive outcomes for nursing staff and patients.
